How do the incentives for represented employees differ from those for non-represented employees?
All employees, retirees and their spouses are eligible for the Quarterly Health Engagement Drawings. Non-represented employees and their spouses are eligible to receive a $200 HealthyMe incentive for completing the health assessment ($50) and a health screening ($150). This replaces the Healthy Behavior Incentive from previous years. In the future the activities to receive the HealthyMe incentive will change. Employees who are part of the Local 17 and Outstate unions have negotiated a $50 gift card for completing a health screening.
+
How will the Non-Tobacco Use Credit be monetized moving forward?
As was done in the past, employees who qualify for the Non-Tobacco Use Credit will receive the credit in their paychecks. The total amount awarded will be spread across each of the 26 pay periods throughout the year. Employees can receive up to $400 for themselves, $400 for their spouses, and $400 for dependents.
+
When is the health assessment due?
Employees will have until Sept. 30, 2011 to complete the assessment. However, employees and their spouses will need to complete the health assessment to take advantage of the new Quarterly Health Engagement Drawing. In addition, non-represented employees will need to complete the health assessment in order to earn the HealthyMe incentive. So the sooner you take the assessment, the better.

Absolutely. RedBrick Health has created a Health Screening Form, which is available on their website. Simply take the form with you to your screening and have your physician complete it. Then, mail or fax it back to RedBrick Health to earn credit for completing a screening. Yes. Energize Your Life has created a Flu Vaccination Verification Form, which you received in the preventive health mailing from Evive encouraging you to get a flu shot. Then, either mail or fax the form to Energize Your Life to have your drawing entries recorded. The Flu Vaccination Verification Form is only required for those who choose to get a flu shot outside of the DTE Energy flu shot clinics.
