There are three ways to complete the health assessment:

1. Online -- Log into www.redbrickhealth.com/login. On the left side of your home page, you will see your Healthy Activities list. Click on "Complete your Health Assessment" to get started.

2. Paper -- Call RedBrick Health at 866.261.7144 to request a paper health assessment be sent to you directly.

3. Phone -- Call RedBrick Health at 866.261.7144 to complete the health assessment over the phone. A RedBrick Support Specialist will go through the health assessment questions and record your responses. Upon completion, they will mail you your Health Assessment report including your health score and next steps.
