  • Your DTE account number, address and contact information.
  • The Surge Protection Plan you’re enrolled in.
  • The following details and documents:

Required information if you have filed an insurance claim:
 

  • Name of the insurer
  • Claim number
  • Agent's name
  • Agent's phone number

Surge Damage Information

Details about the surge event and key information on the damaged products:
 

  • Purchase date
  • Manufacturer/brand
  • Model number
  • Serial number

Digital Copy of Estimate

Clear digital copies of an authorized repair company’s estimate must include:
 

  • Company’s license number
  • Technician’s signature
  • Estimates for each surge-damaged product

Digital Files and Images

Additional digital images if key product-specific details are not on the estimate:
 

  • Model number
  • Serial number

The form must be completed and submitted to DTE within 30 days from the date you discovered damage to your covered items.

Please ensure all required information and documentation is provided to avoid delay or denial of your claim.

DTE may, at its sole discretion, require additional information relating to your claim.
